California bill promotes licensing naturopaths

California's Legislature looks set to license naturopaths, which would make it the 13th state to regulate the profession.
Armed with licenses, naturopaths then would be eligible for reimbursement from health insurers. They could order lab tests for clients, prescribe drugs besides botanical supplements and vitamins, and deliver babies. And they would be able to call themselves doctors.

"This is a red-hot-button issue," said Rebecca Patchin, MD, speaking in her role as California Medical Assn. legislative committee chair. She also is an AMA trustee. "There's an increasing number of allied health providers who are wanting to practice medicine."
An estimated 65 to 200 naturopaths in the state would be eligible for this license, while 1,000 to 2,000 naturopaths would not qualify. The bill would license only graduates from five accredited naturopathic schools in the United States and Canada.
The American Assn. of Naturopathic Physicians estimates that there are 3,000 in the United States who would qualify for licenses if all states allowed this.
The California bill has passed the Senate and is working its way through the Assembly. Policy watchers say it is all but assured that the bill, backed by Senate President John Burton, a Democrat, will pass.
Sally LaMont with the California Assn. for Naturopathic Physicians, who has been working for the bill's passage, said the bill would make consumers safer.
"The bottom line is you need a license to practice medicine, even if it's natural medicine," she said. "Right now anyone can call themselves a naturopath in California."
The CMA is against the Naturopathic Doctors Act and has been able to add requirements for doctor oversight for prescribing and childbirth assistance. Language on minor surgery had yet to be finalized, but it looks like this would be limited to treating minor abrasions and superficial treatments, such as removing warts.
Dr. Patchin said the changes would give naturopaths similar authority as nurse practitioners. But there's a two-year evaluation period for physician oversight of prescribing authority, which raises the possibility that oversight could be removed.
"We hope in the long run we'll have independent prescribing rights," said Beth Wotton with the California Assn. for Naturopathic Physicians. "This gives us a foot in the door."
But the Coalition for Natural Health is one naturopath group that's against the bill. "It's all market-driven," said the group's executive director, Boyd Landry. "Once you let them get their nose under the tent, you'll be dealing with them every year until they get primary care physician status."
The last state to face this issue was Kansas, which voted in 2002 to register naturopaths, but did not sanction licensing or any practice of medicine outside of botanical treatments.
Michael Traub, ND, president of the American Assn. of Naturopathic Physicians, said the issue is likely to come up in Idaho, Missouri, Florida, North Carolina, Washington, D.C., New York and Massachusetts. "Clearly, people have been disenchanted with conventional medicine for some time," he said.
Under current law, practitioners of naturopathy must tell patients they are not licensed physicians and that their services are not licensed.
According to some studies, 43% of Americans have used some form of alternative or complementary medicine and are spending $40 billion a year on such treatments or remedies.
